Region 188 Fall 2008 Registration Instructions:

 

1. Pre-register online at www.eayso.org .  Follow the instructions and ensure that “Region 188” is designated as your desired region. 

 

2. Print and sign 3 copies of the registration form. (Note if you have trouble printing the registration forms, disable your pop-up blocker.  If it still doesn’t work, it may be your particular web browser.  If this is the case please email the registrar).

 

3. If your player is participating in the Fall 2008 season, write the word RETURNING at the top of your paperwork and the name of your player’s coach.

 

4.  Print the sponsor’s service affiliation at the top of the form (i.e., USN , USA , USAF, USMC, USCG, DoD)

 

5. Bring the following to one of the registration events listed below:

•           Pre-registration forms (3 copies)

•           Payment (Check or Cash) in the amount of $55.00/player (checks payable to "AYSO Region 188”)

•           Proof of Age (first time registrants with Region 188 only)

 

 

Final registration date:

Tuesday, 19 AUG 7:00-8:00 PM at the Hickam Theater

U5 and U6 Only - - will accept waitlist registrations for all other age groups.
