Region 188 Spring 2009 Registration Instructions:

 

1. Pre-register online at www.eayso.org .  Follow the instructions and ensure that “Region 188” is designated as your desired region. 

 

2. Print and sign 3 copies of the registration form. (Note if you have trouble printing the registration forms, disable your pop-up blocker.  If it still doesn’t work, it may be your particular web browser.  If this is the case please email the registrar).

 

3. If your player is returning from either the Fall 2009 season, write the word 'RETURNING' at the top of your paperwork.

 

4.  Print the sponsor’s service affiliation at the top of the form (i.e., USN , USA , USAF, USMC, USCG, DoD)

 

5. Bring the following to one of the registration events listed below:

  • Signed ppre-registration forms (3 copies)

  • Payment per player (check or cash) in the amount of $45.00 if returning from Fall Season or $55.00 if player did not participate in the Fall Season. Make checks payable to "AYSO Region 188”.

  • Proof of Age (first time registrants with Region 188 only)

Saturday, 31 OCT 2009/1000-1200/U8 Field behind Mokulele Elementary
Saturday, 7 NOV 2009/1000-1200/U8 Field behind Mokulele Elementary
Saturday, 14 NOV 2009/1000-1400/Hickam Bxtra
